Access to Your Medical Records
View your GP health record online using the NHS App, or request full record access and copies from our team.
View your GP health record in the NHS App
All patients aged 16 and over can view their prospective GP health record online for free using the NHS App or an NHS website account.
Once logged in with verified NHS photo ID, you can see your medications, allergies, immunisations, consultation notes, hospital letters, and test results once reviewed by a doctor.
Request full historical access or copies of your record
If you would like online access to your older historical GP record, or if you need a digital or paper copy of your medical records (a Subject Access Request), we are happy to arrange this for you free of charge.
- • Request online: Submit an administrative request via our online form.
- • Request in person: Ask at reception and bring photo ID and proof of address.
- • Request by email: Email r.quayside@nhs.net with your full name, date of birth, and what records you need.
In line with UK GDPR, we complete Subject Access Requests within one calendar month (and usually much sooner).
Accessing records for a child or someone you care for
If you need to view medical records, order repeat prescriptions, or book appointments on behalf of a child, elderly relative, or person you care for, you can apply for proxy access.
