This is an Oldham based award for individuals working in health or social care that can be nominated by the public / patients for someone that has gone above and beyond.
Quayside Medical Practice sits within the Oldham borough meaning our patients are able to nominate.
You can nominate any health or social care worker that works in Oldham.
This could be for example:
- GP
- Nurse or HCA
- Manager
- Receptionist or Administrator
- Allied healthcare professional
- Focused care worker
- District Nurse
- Social worker

August = 343 missed appointments
September = 357 missed appointments
Missed appointments cost the NHS over £200 millions pounds per year.
Missed appointments mean other patients needing medical help or support can not get an appointment as easily.
Please contact us ASAP if you no longer need your appointment or are unable to attend so we can offer it to someone else.
Patients receive text messages for their appointments and reminders before.
Patients may be contacted by the practice if they repeatedly do not attend to identify issues and prevent this from happening.
Avoid the busy phone lines and book with ease from home. Book routine and same day appointments (same day slots visible from 8am on the day).
Request medication online instead of phoning our busy phone lines. Don't have patient access yet? It is easy to do. Simply register and start ordering your medication. It's that easy!

How does Focused Care work?
Patients are referred by practice staff, local community workers or even the police, when the usual care plan does not appear to be working. The Focused Care Practitioner then works with the patient’s household to begin to unpick situations, assessing need and using local health and community contacts in order to begin to bring stability to an often chaotic situation. They bring together agencies and patients, and also establish accountability for the patient and for the agencies involved, meaning that appointments are attended, practical support is provided and a glimmer of hope and change comes to that household.
