How to Register with Quayside
Join Online
If you wish to register with the practice, please use our new online form. Instead of being directed to the NHS app or being asked to create an account like other practices, which is difficult and can take time, we have developed our very own form that you can complete online in just a couple of minutes - making it incredibly easy to register with us!
Paper Form
If you are unable to complete the Patient Registration form online, please either download & print the form below or collect a paper copy from the surgery.
Providing ID & Proof of Address
Once the registration form has been completed, please attend the surgery with your identification:
- Photo ID (passport or driving licence)
- Proof of address (Utility bill or bank statement)
Registered online? Make sure you send us your photo ID and proof of address with the online form or drop it off at the surgery for our staff to check and take a copy.
