How to Register with Quayside

If you wish to register with the practice, please use the link below.

If you are unable to complete the Patient Registration form online, please either download & print the forms below or collect a paper copy from the surgery. 

You can find your NHS number via this link

Once the registration form has been completed, please attend the surgery with your identification:

1. Photo ID (passport or driving licence)
2. Proof of address (Utility bill or bank statement)

Registered online? You will be asked to email or drop off a copy of your ID back to us.
